CANCELLATION POLICY

What if I decide to cancel? Will I still be able to access my data?

Upon the termination of Subscriber’s full use of the BackChart® Software due to Subscriber’s decision to not continue to make monthly payments (the first day of the month after Subscription Pay is not timely made shall be the “Subscription Expiration”). Subscriber will be provided with 1 year of read only access to Subscriber’s data entered into the BackChart® Software from the date of Subscription Expiration. Subscriber will be able to export & print all previous patient data & reports with read-only access. Subscriber may extend read only access to Subscriber’s data in the BackChart® Software by paying $29.00 on or before the 15 day of each month.

General Data. Following Subscribers termination of read only use of Subscription data, such data shall be maintained offline by BackChart® for no less than seven (7) years. A former subscriber may request copies of such data which request shall be provided manually and billed at BackChart®’s customer work hourly rate of $100 per hour.
